**Returns have to be authorized prior to shipping. For warranty, customer has to provide equipment model and serial number where lamp was installed, lamp serial number, date installed and hours used, and description of problem. Lamp will be sent to Excelitas for evaluation and it takes about 2 weeks for them to do it. Excelitas determines if warranty is valid based on their evaluation.
